Project Adults

Coordinator: Kelly Borden, LSW

Project ADULTS provides a continuum of services for young adults aimed at bridging the gap between school and adult life. Funded by the Mercer County Department of Human Services, Project ADULTS works with students approaching graduation from Mercer High School and students with disabilities referred from other Mercer County public high schools. Project ADULTS staff helps prepare a comprehensive transition service plan.

After graduation, Project ADULTS provides direct services such as counseling, information, referral services, and general service coordination to young adults and their families.

Project ADULTS provides leadership to a countywide organization, the Mercer Area Partnership (MAP), whose mission is improving the transition from school to adult life for young adults with disabilities. The MAP, which draws its membership from the staff of all high schools in Mercer County, representatives of community agencies, family members and consumers, has organized training sessions for high school special education staff around issues of self-determination, and developed educational materials for the business community. Project ADULTS has written a handbook and service directory on transition issues.

Mercer County Special Child Health Services

The Special Child Health Services (SCHS) Case Management Unit assists in the provision of coordinated and comprehensive services for children with special health needs or potentially disabling conditions. Integrated Preschool Project

Mercer County Special Services has been involved in a unique partnership with local school districts to provide a variety of Integrated Preschool opportunities throughout Mercer County. Beginning in 1994, a group of parents, child care providers, child study team members, and directors of special services met to design a grant application to support community-based services for preschool aged children in need of special education and related services.

This program has grown significantly during the last several years. The project designed a three-tiered model of support in the nursery school: consultation to the preschool staff; in-class support to the target children; and inservice development for community-based staff. Project staff include special education teachers, speech, occupational and physical therapists, behavior specialists, classroom assistants, and a supervisor.

The Integrated Preschool Project has answered the mandate to provide a full continuum of services in the least restrictive environment.
